If you are new to our office, please take a few minutes to fill out your new patient paperwork electronically using the link below. If you cannot fill out this paperwork before your appointment time, please arrive at least 10 minutes early to complete it at our office. If you are an established patient at our office, you can skip the new patient paperwork.
Always bring your photo ID, and any Medical Insurance cards, including supplements, with you (yes, even for your vision exam). Most vision plans do not have cards, and we can usually find your plan using your name and date of birth.
Taking any medications? If you’ve got a long list, it may be easier to bring this list with you than to try to remember all of them.
Bring any current prescription glasses or sunglasses you have so the doctor can assess what you are currently wearing, and give you a better idea if an update is needed or not. If you are a contact lens wearer, bring any old boxes so the current prescription can be reviewed.
No problem, just make sure you give our office at least a 2-hour notice so we can make this appointment available for someone else. Remember - we’re reserving this spot just for you. We do charge a fee for any no-shows or late reschedules/cancellations. For those with early appointments, texting our office before we open is also OK to avoid this fee.
